Types of Reports

Different circumstances require different levels of reporting, and different types of reports. We will consider here one method (of many possible) for classifying reports. This method has worked in a number of casinos, and could very well be used in other industries. It involves only three separate types of report: 1.      Verbal report accompanied by written entry in Daily Surveillance Report (DSR) 2.      Procedure Violation Report (PVR) 3.      Incident Report (IR) We will break these down and describe the circumstances of each, how to make the report, and the data which must be included. It should be noted that certain circumstances require two or all three of the reports noted above. . . .
